The Learning Lessons Bulletin features summaries of investigation reports by the Prisons and Probation Ombudsman into deaths in custody. They have been selected for publication by the National Offender Management Service's Safer Custody and Offender Policy Group because of the learning opportunities they provide. The Bulletin also features selected 'Rule 43' recommendations resulting from coroners' inquests into deaths in custody, and the responses to those recommendations.
